Licensed Pharmacist
License Description Applicant must: be 19 years of age or older; Be of good moral character; Be a U.S. citizen or provide verification that applicant is authorized to work in the U.S. as proven by the U.S. Immigration and Naturalization Board; Hold a professional degree from a division, school, college or university department of pharmacy recognized by the ACPE, American Council on Pharmaceutical Education; Have completed an approved practical training program of 1,500 hours under the supervision of a licensed preceptor in a pharmacy recognized by the board as qualified for training pharmacy externs and interns; Present proof of the completion of these practical training requirements by affidavit from the preceptor under whom training is served.
