Lead-Based Paint Abatement Contractor/Consultant
License Description
A Lead-based Paint Abatement Contractor/Consultant is any firm, partnership, corporation, sole proprietorship, association, or other business entity that performs lead-based paint activities in target housing and child-occupied facilities. Requirements: 1) A completed application provided by the Department 2) Current insurance certificate covering lead-based paint activities 3) A letter attesting that the contractor or consultant shall employ only appropriately certified employees to conduct lead-based paint activities, that its employees shall follow the work practice standards contained in Arkansas State Board of Health's Rules Pertaining to Lead-based Paint Activities, and that a supervisor will be present during all lead-based paint activities. 4) Disclosure statement with original signatures and notary stamp 5) Required fee. License fee: $1,200.00
