Conscious Sedation Permit
License Description Dentists. Fees: Application: $500 On-site inspection: $2000 Renewal: $325 Examination Frequency: On-site inspection and evaluation - every 6 years. Exam Locations: For on-site inspection and evaluation the address indicated on CS permit. Experience Requirement: Successful completion of a course in administration of conscious sedation consisting of at least 60 hours of instruction and satisfactory completion of at least 20 cases of administration of conscious sedation. The course must comply in all respects with the Guidelines for Teaching the Comprehensive Control of Pain and Anxiety in Dentistry of the American Dental Association. Average Time to Process Application: 5 days. Renewal Period: Biennially (every second year). Special License Requirements: Renewal of a CS permit requires satisfactory completion of 15 units of continuing education in courses related to the administration of conscious sedation and medical emergencies.
