License Description Qualified applicants may apply for a combination license (esthetician, eyelash technician and/or nail technician). The combination license application fee is $100 for two licenses or $200 for all three. Regardless of the number of licenses you apply for, the renewal fee will be $100 every two years. Applications are only accepted online. Please select this link to apply online. After you have registered for an account, please select 'Initial Application' at the bottom left of the screen and then expand the 'Appearance Enhancement' grouping and select the 'combination license' category. Please select this category ONLY if you intend to and qualify for the licenses you wish to apply for.
