Respiratory Care Practitioner Temporary Permit
License Description In order to obtain a temporary permit, an applicant must provide the following documentation: A completed application with photograph attached. Applications are only accepted online. Please select this link to apply online. Once logged in, please select the 'Public Health Practitioner' grouping and then 'Start' Next to 'Respiratory Care Practitioner Temporary Permit' As part of the application process, you will need to upload: A copy of your diploma or other document such as a letter from the program documenting that you have completed the program; A completed 'Supervision Verification' form. Once an application has been received and reviewed, the applicant will be notified by email.
