License Description In order to obtain licensure, applicant must submit transciprts directly from an accredited university, a certification from thir educational institution stating the applicant has completed 400 hours of direct supervised clinical experience, certification that applicant has completed a clinical fellowship, official ETS-PRAXIS socres, a Certification of Claninical Competence from ASHA, and a criminal background check.
