License Description GENERAL JOB DESCRIPTION Performs examination of the human eye by acceptable means including the prescribing of lenses, prisms or vision training to correct visual defects. Therapeutic Certified Optometrists may write prescriptions for oral and topical drugs. EDUCATIONAL REQUIREMENTS Minimum of three (3) years of college, with four (4) years of optometry school with a minimum of twelve (12) semester hours (transcript credit) in general and ocular pharmacology. EXAMINATION REQUIREMENTS Parts I, II & III of the National Board of Examiners in Optometry as well as TMOD. Kentucky will give only a State Law Exam and an Oral Orientation. For application and dates, contact the regulatory agency. GENERAL INFORMATION for LICENSURE or CERTIFICATION Initial and annual renewal licensure fees are $200.00 for Kentucky residents and $400.00 for non-residents. Applicant must be at least eighteen (18) years of age. There are no residency or health requirements. License must be renewed annually before March 1. For renewal, evidence must be presented reflecting compliance with the Continuing Education Requirements. For specific information regarding reciprocity with other states, contact the regulatory agency. CONTINUING EDUCATION REQUIREMENTS (after licensure or certification) Must complete eight (8) hours annually for those NOT drug certified. Fifteen (15) hours per year for those TPA certified.
