License Description
GENERAL JOB DESCRIPTION Counsels and provides medication related information to patients, physicians, and other practitioners on drug usage. Provides drugs and medical supplies, compounds prescriptions, prepares pharmaceutical preparations, maintains legend drug distribution as required by state and federal regulations. Site of practice includes hospital, nursing home, community, nuclear, medical service representatives, teaching, manufacturing and repacking, research and government. EDUCATIONAL REQUIREMENTS Must be a graduate of an approved (all Pharmacy schools and colleges approved by the American Council on Pharmaceutical Education are hereby approved by the Kentucky Board of Pharmacy) college of pharmacy B.S. Degree in Pharmacy or an entry level Doctor of Pharmacy Degree and must file proof satisfactory to Board, substantiated by affidavits of completion of approved internship under the supervision of a currently licensed pharmacist, who is a certified preceptor. Must complete no less than one hour HIV/AIDS continuing education approved by the Kentucky AIDS Education Program, Cabinet for Health Services, prior to examination. EXAMINATION REQUIREMENTS Contact the Kentucky Board of Pharmacy for application procedures. Examinations scheduled two (2) times yearly. Location of examinations conducted within the state as designated by the Board. Type examination; knowledge, education, competency of applicants. Must score at least 75% on operative / practical examinations; and at least 80% on jurisprudence. Examination fee is not to exceed the amount of $150.00 (does not include any direct costs for test materials and supplies). Should you fail the examination, the Board Examination shall be considered as composed of two (2) segments: (1) The NABPLEX Examination, and (2) The Kentucky Examination.
