Liquor liability insurance in the exact name and covering the complete license period. Workers Compensation insurance proof. New establishments require physical inspection of premises. Must be 21, no felonies, liquor law convictions, or revocations within 5 years. Must be of good moral character and reputation.
No license will be approved or released until the $20 Retailer ID Card fee is received.
