License Description Applicants must: 1) Be over 21 and of good moral character. 2) Be a U.S. citizen, or legally entitled to live and work in the U.S.. 3) Have graduated from a dental school accredited by the ADA. 4) Pass the National Board of Dental Examiners examination with an average score of at least 75%. 5) Pass one of the following clinical examinations: 5a) Western Regional Examining Board (WREB) exam, or 5b) ADEX-Dental Exam (must have been taken after October 1, 2007). 6) Submit the results of a National Practitioner Data Bank self-query (on-line @ www.npdb-hipdb.com.) 7) Pass the online Nevada jurisprudence examination. *Notes: ADEX = American Board of Dental Examiners; ADLEX = American Dental Licensing Examination; PBIS = Professional Background Information Service; CCBS = Complete Credentialing & Background Services. Reciprocity: Nevada offers licensure by examination. Fees may be subject to change. Fees are: application packet - $25; license application by WREB or ADEX - $1200; biennial license renewal--$600; self-query - $16.
