Boxer, Kickboxer and Mixed Martial Artist (Professional Contestant)
License Description Applicants must be at least 18 years old and have a current Federal ID card (if professional boxer or mixed martial artist.) They must submit results from a dilated ophthalmologic exam done by an Ophthalmologist; a yearly sports physical (Physical Examination Report - Professional Boxer/Unarmed combatant) including an original laboratory report with the applicant's name and the date the HIV, CBC, Hepatitis B and Hepatitis C tests were taken. The HIV test must be done within 30 days of submitting all requirements to become licensed, the CBC and Hepatitis tests can be done within the calendar year. Also required are the results of an MRI and MRA done within the last 5 years. Applicants must submit to drug testing if required by the Commission. There are additional requirements for the following individuals: 1) Amateur boxers making their professional debut. 2) Boxers over the age of 35 and younger than the age of 18. 3) Boxers with no activity for 36 months. 4) Boxers who have competed in over 425 professional rounds. Fees are: application--$25, plus actual costs for a physical examination by a qualified physician and for laboratory tests.
