Home Podiatric PhysiciansPodiatric Physician Applications Podiatric Physician Applications **NOTICE** In order to comply with federal law, the West Virginia Board of Medicine is obligated to inform each applicant or licensee from whom it requests a Social Security Number that disclosing such number is MANDATORY in order for this Board to comply with the reporting requirements of the National Practitioner Data Bank. The application to practice podiatry in West Virginia is to be personally completed with all required information and attestations by the applicant. If you are applying for licensure for the first time or wanting to reactivate a previously held license that has not been active or inactive for more than one year please click here to access the on-line application. For additional information regarding reactivating your license, changing from inactive to active status, or reinstating a license that has been expired for less than one year please contact the Board office at 304.558.2921,and select option 1 from the menu. You will find a copy of the application instructions below for your review prior to accessing the on-line application. The $400 application fee is paid on-line at the end of the application process and is non-refundable, so please verify your eligibility for licensure by reviewing the separate "Requirements" information tab.
