License Description Resident Application Requirements According to our Rules §24-1-13, all graduate medical trainees shall secure an educational training permit if they have not already applied for a West Virginia license. He/She shall submit a completed application. You may access the online application for Osteopathic Educational Permit by using the Continue button at the bottom of the page. With the application, you will also need to: mail or upload a copy of your Medical School Diploma submit a $100.00 application fee submit a processing fee that is 2.25% of the total fee Should you have any questions, please do not hesitate to contact our office.
