Occupational Therapy Assistants (OTA)
License Description
GENERAL INFORMATION - PLEASE READ Licenses are valid from the date issued to December 31 of the following year and are subject to biennial renewal. Application must be completed in its entirety. Failure to do so may result in delay and/or denial of license. Applications are completed online through Certemy, our licensing management software provider, and include a number of self-paced steps to be completed and submitted for verification and approval. A recent photo is required to be uploaded as part of the online application. A completed Supervisory Statement, signed by a supervising OTR, is required to be uploaded as part of the COTA application for those applicants who have secured employment in WV. If applicant has previously been licensed, in WV or any other state, but does not hold a current license or NBCOT certification, and has not been a practicing clinician within a period of 2 years, you are required to submit proof of 24 hours of continuing competency contact hours within the 2 years preceding the application. This does not apply to new graduates. Please contact the Board for additional instructions. To insure compliance with Federal Law, the WV Board of Occupational Therapy is obligated to inform each applicant that reporting of the Social Security Number on licensure application is mandatory according to W. Va. Code §30-1-6 (d). Once all application steps are completed and approved, a WV license number will be assigned and Certemy will generate a digital license card which can be found in the digital wallet of your Certemy account. The Board will mail a Certificate of License and set of two license cards to each new licensee. It is the responsibility of the applicant to be informed of West Virginia's Occupational Therapy Practice Act and Legislative Rules, copies of which will be included with license. LICENSURE FEES The initial license fee for an OT is $150. The initial license fee for an OTA is $100. Licensure fees will be collected through the online application using the State of WV's e-Gov payment processor. A copy of your payment receipt must be uploaded as part of the online application. License fee is not refundable. READY TO APPLY? Applicants must self-enroll into an application and create a Certemy account. When creating your Certemy account, it's highly recommended you use a personal email address. This prevents disruption should you change jobs or if your employer uses firewalls which may block important reminders from Certemy about your license.
